What part is this? Leaking coolant

This T looking joint is leaking and coolant is flowing down the rubber line all over the pavement. Moderately sized leak so I'd like to fix this ASAP. I tried to find this part on RockAuto but my search came up short. Can anyone tell more specifically what this part is and how I could go about getting a replacement?

Additionally, I can't tell how it is installed. I can see the holes but I don't see a clamp or fastener. Are they just pushed in?

That's what delivers coolant to your front and rear heater core. It's a very common failure. The part is not available separately - Ford sells it as a $200+ assembly. You can source a wye from a parts store and cut the current one out and replace with hose clamps. Some do it in place - I pulled the whole thing out to get more room.
